OOPH - Oophorectomy (ovary and tube removal)

Cancer Specifications
Stage 1b; IDC 1.3 cm.; Areas of DCIS identified; No node involvement.
ER+ 100%; PR+ 85%; Her-; Grade 3; ONCO score 41
BMX on 5/24/10.
Reconstruction with free tram flap.
Start Chemo 7/22/10;
Dose Dense A/C x 4 every other week; T x 4 every other week.
